Insured patients 1) Bring your insurance card with you to all appointments. 2) Be prepared to make your insurance co-payment at the time you receive services. Your co-payment amount is typically found on your insurance card. 3) We will file insurance claims on your behalf. 4) You are responsible for costs not covered by insurance. Patient account representatives are available to discuss payment options. Uninsured patients 1) You are responsible for full payment of your bill. 2) If you pay your bill within 30 days of the first billing date, you may be eligible for a prompt payment discount. 3) Financial assistance is available on a case-by-case basis. All other payment options must be exhausted first and required financial information provided to the business office.
