Cardiac Rehab is monitored exercise therapy and education to people who have experienced a heart attack, heart surgery, or heart disease. The goal of Cardiac Rehab is to assist each patient to gradually increase physical capacity through exercise and to reduce risk of further heart disease. RETURNING YOU TO AN ACTIVE LIFESTYLE. Cardiac Rehab is divided into three phases designed to meet individual needs: Phase I - Exercise and education program which begins while you are in the hospital. Involves nursing, physical therapy, occupational therapy and consultations from dietary, pharmacy and social services as needed. Phase II - Follow up of education and activities, which begins 1-4 weeks after hospital discharge. Exercise sessions offered three times per week for 4-12 weeks that consist of warm-up, approximately 30 minutes of electrocardiogram monitoring exercise and cool down, relaxation period. Physicians referral is necessary for entrance in to Phase I and II. Phase III - For people who complete Phase II which involves a one-hour session one to three times per week. The goal is to improve or maintain cardiovascular fitness as well as to reduce risk factors for future heart problems. The program is coordinated by Keri Weick, RN.
