The Caregiver Award

The Heart of America Medical Center, including the Long Term Care and Haaland Home are pleased to announce an exciting opportunity for your consideration. As you may already know, our facility is a member of the ND Long Term Care Association. This organization offers a statewide Caregiver Award, and only residents or their families are eligible to nominate long term care employees for this unique honor.

The Caregiver Award recognizes individuals working in the long term care profession who substantially enhance the quality of life for residents in North Dakota long term care facilities. These individuals care for you or your loved ones. Here is a way for you to show you care for them.

The recipient of the 2003 Caregiver Award will be recognized during the annual NDLTCA Convention in May. A cash award, recognition plaque and Caregiver Booklet are presented to the recipient. All nominees will receive a Certificate of Outstanding Achievement and a Caregiver Booklet from the NDLTCA. This Caregiver Booklet contains excerpts from every nomination and is also distributed to you, all facilities, state agencies and legislators.

To obtain an application for the NDLTCA Caregiver Award, contact Administration at the Heart of America Medical Center in Rugby 800 Main Ave South or at 701 776-5261 Ext. 218. You may also find an application on-line at www.ndltca.org/links.html. We hope you will consider nominating any member of our staff who delivers outstanding care to your family. The application deadline is February 15, 2003
