Before you become a resident at our facility, your needs will be assessed. Your Stay Starts with Assessment Before you become a resident of Heart of America Long Term Care, your needs will be assessed by an interdisciplinary team including your physician and nursing, social services, dietary and activities staff members. The services you will receive when you become a resident will be determined via the assessment process. Your needs will be reassessed four times a year or as needed.
