Our social workers are here to help with discharge planning. Discharge Planning Heart of America Medical Center social workers assess the needs of patients being discharged from our facilities to their homes. Discharge plans frequently include services like Meals on Wheels, Homemaker programs and Home Health Care programs. We also can provide patients and their families with information on living situations to meet changing physical, social and emotional needs.
