Hospice Team
Heart of America Hospice services are delivered by a team of physicians, nurses, social workers, counselors, certified nursing assistants, clergy, therapists and trained volunteers.
Initial assessments and individualized care plans are completed by our medical director, hospice nurses, social workers, spiritual care advisor and the patient's attending physician.
We encourage patients to continue seeing their primary care physicians after they are admitted to hospice.
As patients continue in hospice, care plans are updated based on input from patients and their families, as well as input from their physicians, pharmacists, dietitians, occupational therapists, physical therapists, nursing assistants and hospice volunteers.
